Yumlish

Lakeshore worked with Yumlish to adapt their diabetes prevention and self-management programs for people with disability, which included providing a review Yumlish’s program materials, creating an inclusion addendum, providing training to coaches and staff, and hosting workshops with people with a disability and diabetes. We also provided support to Yumlish in its development of a web-based platform, including training for coaches and supplemental content for people with disabilities.

Special Olympics International

Special Olympics International (SOI) partnered with Lakeshore to create printed and digital guidance materials for facilities, equipment and training for athletes with high support needs. Three new guides were developed with participant, coach, volunteer and staff input:

  1. Special Olympics Sport for Athletes with High Support Needs: Guidance for Volunteers
  2. Special Olympics Sport for Athletes with High Support Needs: Guidance for Facilities
  3. Special Olympics Sport for Athletes with High Support Needs: Guidance for Adaptive Equipment

SOI also consulted with Lakeshore to adapt the National Diabetes Prevention Program and update inclusion domains for people with intellectual and developmental disabilities.

Blue Cross Blue Shield of Alabama Caring Foundation

University of Alabama at Birmingham – My Health, My Life, My Way

My Health, My Life, My Way is the first inclusive, chronic condition self-management program that uses a person-centered approach. Developed by Lakeshore, the program includes content covering self-care and goal setting, basics of diabetes, healthy eating, physical activity, resiliency, finances and more. In addition to development of the program which included focus groups and regular stakeholder meetings, Lakeshore also led marketing and recruitment efforts, creating a program website and successfully recruiting over 250 users. Lakeshore also managed health coaching for the program with 13 health coaches offering over 2,286 virtual coaching sessions.

Girls on the Run

Lakeshore partnered with Girls on the Run to adapt the program’s curriculum and coach training to ensure accessibility for participants with cognitive, sensory and physical disabilities.

University of Pennsylvania School of Dental Medicine

Lakeshore partnered with the University of Pennsylvania School of Dental Medicine to serve as the lead subject matter expert in a year-long partnership to develop a collaborative approach to identifying and removing barriers in dental care for people with disabilities in the Greater Philadelphia area. Key findings highlighted the need for addressing accessibility, transportation, communication and patient support.

National Association of Chronic Disease Directors (NACDD)

Lakeshore is partnering with NACDD to advance the prevention of commercial tobacco use and cancer in populations experiencing tobacco- and cancer-related health effects with a focus on people with disabilities (PWD), including U.S. veterans. The Disability Network is supported by a five-year, CDC-funded cooperative agreement and brings together a consortium of nine nationals organizations to increase the delivery of tobacco control and cancer prevention strategies and related interventions.

Lakeshore has worked with the American Association of Health and Disability (AAHD) and the All of Us Research Program as a part of the Disability Community Engagement Partner Project. Through this work, we have shared information about the All of Us research program to our network through digital communications and engagement events.
